Epidemiology:

Rare tumors with decreasing incidence in the USA, according to SEER data.

Pathogenesis:
Most cases are sporadic but there are specific risk factors such as primary sclerosing cholangitis, biliary tree cystic disease and hepaticolithiasis.

Preoperative planning:
Preoperative planning includes the patient`s performance status assessment, staging of the disease as well as assessment of the liver function and functional liver remnant (FLR). Drainage of the biliary tree if necessary, imaging to rule out metastatic disease, and classification of the tumor are necessary steps preoperatively. Imaging includes noninvasive diagnostic CT and MRI/MRCP. Invasive tests like ERCP and PTC maybe also of great importance in setting the diagnosis through FNA biopsies.

Surgical management:
Resection of the involved intra- and extrahepatic bile ducts, as well as the ipsilateral liver, is the standard of care for suitable patients with R0 surgical margins. At the time of operative exploration, 20 - 50% of patients are found to be unresectable.
Median survival and 5-year survival among patients with a negative margin (R0) resection range from 27 months to 58 months and from 27% to 45%.
An alternative way to manage surgically these patients, in selected cases is liver transplantation with good outcomes.

RETIREMENT of prof. Alfredo CARRATO – Announcement!

The VISION consortium would like to announce the recent retirement of Dr Alfredo Carrato as head of the Medical Oncology Department of the Ramon y Cajal University Hospital. Alfredo has been at the Ramon y Cajal Hospital since 2008 and was Scientific Director of the Ramon y Cajal Health Research Institute (IRYCIS) from 2015-2019. Alfredo has more than 35 years of experience in the development of clinical trials and translational research, and is an internationally recognized expert in the diagnosis, treatment and research of pancreatic cancer. With over 350 publications in peer-reviewed journals and funding for over 40 research national and European projects. Alfredo will continue his role as Emeritus Professor of Oncology at Alcala University and Head of the Molecular Epidemiology and Predictive Markers in Cancer Group at IRYCIS, as he is the PI of many ongoing national and European funded projects. However, he will start to take a step back in order to enjoy his well-deserved retirement. This will be a slow and steady process, as Alfredo has and continues to be a huge part of the pancreatic cancer community and still holds several key positions in Pancreatic Cancer Europe as Chairman, the Spanish Digestive Tumor Treatment group (TTD) at The Executive Committee and the Spanish Biomedical Research in Cancer Network (CIBERONC) as Pancreatic Cancer Research Group Principal Investigator at IRYCIS. Alfredo once said that “pancreatic cancer is a hard bone to row”. In fact, the legacy that Alfredo has left for pancreatic cancer researchers worldwide is great and we must try to attain his excellence in the field and his collaborative and selfless philosophical approach to the fight against pancreatic cancer. Alfredo is a much loved and well respected member of the pancreatic cancer clinical and scientific community and we would like to wish him all the best for the future.
